Ageing Well - Engagement Opportunity - NHS Nottingham & Nottinghamshire

NHS Nottingham and Nottinghamshire are carrying out an engagement exercise to engage with older adults and carers about how they receive healthcare and what matters to them. We are committed to engaging with the population of Nottingham and Nottinghamshire to understand peoples' needs as they age, as well as those who support them and to build that into how we design services.

Our aim is to be responsive to our ageing population and proactively and focus on ageing well and enabling citizens to live longer healthily.

We are looking at how best to try to prevent or slow deterioration of health and "frailty". Much of this is not medical, but may be about other factors, like supporting independence or avoidance of social isolation. We are also working on better identification of health issues and reducing repeated appointments and admissions to hospital. Additionally, we want to understand how best to help people multiple health conditions. The final aspect is end of life and supporting choice and control in planning for end of life and support with compassion and dignity according to a persons' wishes.

It is not necessary to fill out each section of the survey if you do not feel like it is relevant to you. The first question in each section will ask if you wish to complete the section.
