Consent form for COVID-19 testing in secondary schools and colleges

We will be conducting testing for our students upon their return in January 2022, as we did in September. These tests will be done during the school day. If you gave consent to the testing previously and you are happy for that consent to be used, then you do not need to submit consent again.
